MATT FOSTER LOSS PREVENTION DEPARTMENT OSHA Form330A and 300 Log N HADA WCT has completed and mailed out the OSHA Form 300A and 300 Log for 2020 to NHADA WCT members who are required to post it. ATV, motorcycle and powersports dealers and members who had on aver- age less than 10 employees in the calendar year are not required to comply with the OSHA record keeping require- ment. As a reminder, the 300A needs to be signed by the owner and posted for all employees to view from Feb. 1, 2020 through April 30, 2020. Unless directly requested by OSHA, no forms need to be submitted, but the 300 log and annual summary shall be kept on file for at least five years following the year that the data covers. If an injury or illness needs to be edited or changed, you should go back to the 300 log and make the appropriate changes. Any NHADA WCT member with 250 or more employees or an automotive parts store, tire store or accessory store must file electronically with OSHA. March 2, 2020, is the deadline for electronically reporting your OSHA Form 300A data. The NHADA WCT will file electronically for our members, but we need your written or e-mailed permis- sion to do so, along with the average number of employ- ees employed by you in 2019 and the average number of hours worked in 2019. In addition to the continued recording of injuries, any work-related fatalities need to be directly reported to your local OSHA office (800-321-6742) within eight hours, and any work-related in-patient hospitalizations, all ampu- tations, and all losses of an eye need to be reported to OSHA within 24 hours. As with any injury, the New Hampshire Employer’s First Report of Occupational Injury or Disease (Form 8WC) MUST BE FILED with NHADA. The First Report meets the requirements of the OSHA Form 301 for first reports.
